Adoption Application

Please note that questions left unanswered, or partially answered, are grounds for denial of any applicant. I take these adoptions very seriously, and expect you to, as well. Remember - this is an APPLICATION, not a guarantee, and you WILL be judged by how you complete these responses. (Suggestion - use your spell checker, and don't use "IM" language).

PLEASE READ THE SAMPLE ADOPTION CONTRACT PRIOR TO SENDING THIS FORM!!

Please note: Your application may be forwarded to other reptile rescuers in event that they have an animal you could be qualified to adopt. If this is not acceptable to you, please indicate such in the field below where it asks for "additional info". Thank you! We will release your info only to verified rescues that we have an ongoing cooperative relationship with.
